Regulatory Blind Spots and the Explosion at AES

On October 10, 2025, building 602 at Accurate Energetic Systems (AES) in Hickman, Tennessee, exploded with over 24,000 pounds of explosives inside. The explosion resulted in the deaths of 16 employees, demolished the building, and was heard from miles away. The U.S. Chemical Safety Board quickly launched an investigation into the accident, which shows how serious this situation really was.
Building 602 contained TNT and produced commercial mining boosters made from very hazardous materials. Employees had expressed concerns about safety and said they felt uncomfortable because of limited training. Despite these risks, the building had not been inspected in about twenty years. This resulted in an issue, as the building fell into a regulatory gap. Several agencies were involved; however, none took responsibility for monitoring the building that eventually exploded.
At least three agencies were connected with AES, but each of them supervised different parts of the facility, and even those responsibilities were limited. The Bureau of Alcohol, Tobacco, Firearms and Explosives (ATF) managed storage and security, not worker safety. The Department of Defense (DoD) examined only the buildings tied to military production. Building 602 produced commercial mining boosters, so it was not part of their inspections. Tennessee OSHA (TOSHA) was responsible for workplace safety but only visited the site once in twenty years. Since each agency focused only on its own role, none of them were checking the building that exploded.
When comparing this situation to other high-risk industries, it highlights the problem even more. The fireworks industry typically involves a few different groups that handle safety. These safety checks include state fire marshals, OSHA, and local fire code authorities. However, incidents still happen, such as a fatal one in Hawaii in 2011 during a fireworks disposal. The fireworks industry has clear roles for who is responsible for safety. Since each group has its own job, it is easier for safety problems to be caught before they get worse.
Mining is another good comparison because it shows what consistent supervision looks like. The Mine Safety and Health Administration (MSHA) is the main agency responsible for overseeing mines. Their inspection schedule is clear and states that underground mines must be inspected four times annually, with surface mines being inspected twice per year. Mining is still hazardous, and accidents still happen. The difference is that regulators are required to show up regularly. That consistency helps catch hazards before something goes wrong, which is very different from what was happening at AES.
Another problem is that OSHA is severely understaffed across the country. A Statista study showed that, with its current number of inspectors, it would take decades for OSHA to visit every U.S. workplace even once. This shortage explains how hazardous sites can go a long time without being checked. It does not excuse the lack of oversight at AES, but it shows that the existing system is not designed to check every site as often as it should.
The explosion at AES shows what can happen when responsibility is unclear. High-risk industries need at least one agency that is clearly in charge of safety and inspections, instead of several groups assuming someone else is handling the job. A more focused system would make accountability easier and help prevent these hazards from being missed. Creating that kind of structure would lower the chances of disasters like AES happening again.
